Visitors from every corner of Eorzea come to Ul'dah to partake of the city's famed recreation, most notably the fighting arenas and gambling halls.

I agree, but I still would like to see a little more PvP than FFXI.

1. 100% player consent. No zone or area that will automatically turn on your PVP option. Give players the option to turn it off and on, in any zone if they wish minus major cities. If there is a zone for PVP make it an instance. With no requirement, reason, or benefit for a PVE player to have to join. Don’t put PVE quests in that zone, or make a PVE character have to travel thru it. Completely optional, just PVP.
2. DO NOT EVER balance PVE skill/abilities around PVP players comments, post, logic, rant, overpoweredness, or crying / nerd rage. If adjustments need to be made for PVP abilities, then make all the changes you want, but leave the PVE aspect the same. Basically once you turn on your PVP option, all your stats/skill/abilities us the PVP numbers/adjustments.
3. Rewards - This could be tricky, should PVP have rewards? I do not care if they do or don’t. But if I was a big PVP’er and was good at it, then I would enjoy some form of a reward system. The key again is that no matter what the reward is, weapons , armor, new skills, or better version of current abilities , it can’t be used in PVE. PVP rewards stay as PVP only items. You can’t use them in anyway for PVE. When you turn off your PVP option all PVP rewards are unequipped and go in your bag. That way there is no reason for a PVE only player to want those items.

I came to FF to also get away from the amout of pvp orientated games that have come out. I dont want it forced on me or made to be exposed to it. If I wanted pvp I would play Aion.

While I agree with most of your thoughts, even the idea that the items are not usable outside of PVP. I must disagree that you can not show them off outside of PVP. If you're a PVE player and you see those items and desire to have them then so be it. People will weigh out the need of obtaining items from PVP and actually having to jump through the hoops to get it. Also, I believe it would be better to bring people to PVP. What would the benefit be for doing PVP if there were no reward at all? Aside from the satisfaction of fighting other players. This is the reason that FFXI PVP was mostly failure. Except for Ballista which helped with region control.

I doubt that'll happen, it's probably going to be similar to FFXI, I don't know how difficult it was to get in but considering ease of access is XIV's motto I wouldn't think access is going to be a problem.
I like the card dueling idea too, integrate Tetra Master into the game itself would be both awesome and get an edge on Eq/Eq2's card game.
